Mr. Sammie "Sam" Davis Cook a resident of Independence, LA passed away on Wednesday, April 3, 2019  at the age of 76.  He was born on March 31, 1943 to the late Sammie Davis Cook, Sr. and Lorraine Cook Scott in Watson, Louisiana.

Sam is survived by his wife of thirty-nine years, Letha Owens Cook, two children, Tyra Cook-Johnson (Wayne) and Christian Cook and one stepdaughter, Alexa Hookfin; two grandchildren, Demetrius Hookfin, Jr. and Demetriona Hookfin; two great grandchildren Alexandria and Aubrey Hookfin; three aunts, Alfreda Robertson, Hazel Donald and Irene Brider  and one uncle, Tillman Cook.

He was preceded in death by his parents, Sammie and Lorraine Cook Scott, three brothers: Dority Berry, Eugene Cook and James Cook, and one grandson, Jeramie Hookfin

Relatives and friends are invited to attend the visitation on Wednesday, April 10, 2019 from 9:00-10:45, followed by the celebration of life service at 11:00 at Faith Temple C.O.G.I.C. Church, 11605 Fontana Road, Independence, Louisiana. Interment will follow  in Mt. Olive Baptist Church Cemetery, Independence, LA. Arrangements entrusted to Richardson Funeral Home in Amite, LA.
